By contributing to this project, you will help establish:
A Mini-Health Clinic:
A culturally sensitive medical home offering primary care, chronic disease management, preventive screenings, immunizations, health education, mental health counseling, and referrals to specialty care. Our clinic will ensure that uninsured and underinsured community members, including immigrants and refugees, have access to compassionate and affordable physical and mental health services.
A Senior Care Program:
A nurturing space for our elders where they can receive personal case management assistance, nutritious meals, and transportation services. Our program will offer engaging recreational, educational, and social activities designed to promote independence, reduce isolation, and enhance quality of life for seniors and adults with disabilities.
